Targeted Case Management
Here at New Pathways, we offer mental health-targeted case management services to support eligible individuals (youth and adults) in accessing and coordinating with medical, social, behavioral, educational, vocational, housing, and other essential services they need to improve their quality of life. We believe that the right support can assist individuals with overcoming challenges to live healthy, fulfilling lives. Our dedicated case managers provide skills and advocacy for our clients to ensure that all individual needs are met to obtain happiness and independence.
Mental Health Rehabilitation Services
Mental health rehabilitative services are defined as providing assistance in maintaining or improving functioning and may be considered rehabilitative when necessary to help a person achieve a rehabili- tation goal as defined in the treatment plan.
Mental health rehabilitative services may be provided to a person with a serious mental illness as defined in the latest edition of the American Psychiatric Association’s DSM.
Mental health rehabilitative services are age-appropriate, individualized, and designed to ameliorate functional impairments that negatively affect any of the following:

Mental health rehabilitative services are age-appropriate, individualized, and designed to ameliorate functional impairments that negatively affect any of the following:
- Community integration
- Community tenure
- Behaviors resulting from serious mental illness (SMI) or severe emotional disturbance (SED) that interfere with a person’s ability to remain in the community as a fully integrated and functioning member of that community
Mental health rehabilitative services may include:
- Medication training and support services
- Psychosocial rehabilitative services
- Skills training and development
- Crisis intervention services
- Day programs for acute needs
Mental health rehabilitative services may only be provided by a member of the person’s therapeutic team. The therapeutic team includes a sufficient number of staff to adequately address the rehabilitative needs of persons assigned to the team.
Team members must be appropriately credentialed and have completed required trainings to provide the full array of component services, have regularly scheduled team meetings either in person or by teleconference, and every member of the team must be knowledgeable of the needs and the services available to the specific persons assigned to the team.
Mental health rehabilitative services may be a benefit for persons residing in a nursing facility (NF) when medically necessary as determined via a uniform assessment protocol and determined through pread- mission screening and resident review (PASRR) to require specialized services.
Autism Services
ABA is one of many services available for Medicaid members, age 20 or younger, who have a diagnosis of autism spectrum disorder (ASD). ABA is a treatment approach provided by specialists called licensed behavior analysts (LBAs). If your child is eligible, ABA can help: Encourage positive and adaptive behaviors. Apply skills across everyday settings so they can improve their health, safety or independence.
